Claim.MD is a clearinghouse, not a full billing system, and it is on this list for billers who already have practice management software and want a cheaper, clearer connection to payers. Plans are published: Basic costs $30 a month and charges per transaction, Small Volume costs $60 for 100 claims, ERAs and eligibility checks, and Unlimited costs $120 for unlimited claims and ERAs plus 1,000 eligibility checks, with no setup or per-provider fees. Claims can be keyed into CMS-1500 and UB-04 style forms with real-time validation or uploaded in formats from 837P and 837I to CSV and print image. Rejected claims come back for correction, ERAs download in 835, spreadsheet or XML, and eligibility runs through the portal or an API. It scores low on features because it has no scheduling, A/R worklists or denial analytics.
