10 Best EHR Software for Small Practices of 2026

By the TopToolAdvisor research desk · Updated November 6, 2026 · Next review December 21, 2026 · 10 products scored

Our pickathenaOneIndependent physician practices that want billing work taken off their staff and prefer to pay a share of collections instead of a fixed license.8.7

Our pick in brief

athenaOne is the strongest pick for small practices that want both software and people working their claims, paid as a share of collections. Tebra is the most complete all-in-one platform, adding online booking and reputation tools to EHR and billing. RXNT is the best value, with published prices and unlimited claims on its Full Suite. Elation Health is the leader for independent primary care and direct primary care, and CharmHealth is the cheapest way to start with its free edition. Practice EHR, eClinicalWorks, DrChrono, Practice Fusion and Amazing Charts each fit narrower needs, from AI-heavy tiers to a locally installed EHR.

Quick answer

athenaOne is the best EHR software for small independent practices in 2026 that want billing support built in, priced as a share of collections; Tebra suits practices that want EHR, billing and patient marketing on one platform, and RXNT suits practices that want published prices, with its Full Suite including billing at $335 per provider per month. CharmHealth is the cheapest start, with a free edition for practices under 50 encounters a month and a Provider Plan at $200 per provider per month.

This list is for independent physician practices with one to about ten providers choosing an EHR that also handles billing: claims, eligibility checks, payment posting and patient statements. We compared documented features and published prices for 10 products and scored each on features, ease of use and value. Prices are the cheapest paid plan in US dollars as shown on the vendor's own page on the date we checked. Several large vendors quote only after a demo or price as a percentage of collections, so they are marked as pending. Check whether a plan includes billing software, billing services or neither before comparing prices.

We could not confirm a US price yet. Check the athenaOne pricing page.

athenaOne ranks first because it pairs a full EHR with the billing work itself, not only billing software. The suite covers clinical documentation with AI tools inside the encounter, medical billing and practice management, and patient engagement through a portal, mobile app, telehealth and automated wellness outreach. On the revenue side, athenahealth says its rules engine and billing experts fix claim errors before submission, certified coders capture the care delivered, and a specialist team handles prior authorizations. That combination suits a small independent practice without a dedicated billing department. Pricing is a percentage of collections with no published rate, so you need a quote, and a growing practice should model what that share costs against a flat per-provider fee. athenahealth also states there are no long-term contracts and that customers can leave with all their data, which lowers the risk of trying it.

    Strengths

    • +Billing experts and a rules engine catch claim errors before submission
    • +Pricing tied to collections, so the vendor is paid when the practice is paid
    • +No long-term contracts, and you can take your data when you leave

    Drawbacks

    • −No published price; cost is a custom quote based on collections
    • −A percentage model can cost more than a flat fee as collections grow
    • −The service-heavy model means less direct control over the billing workflow

We could not confirm a US price yet. Check the Tebra pricing page.

Tebra is the most complete all-in-one option for a small practice that wants to run clinical, billing and growth work in one place. The cloud EHR covers patient history, e-prescribing, telehealth, labs, scheduling and MACRA and MIPS support, and AI Note Assist drafts clinical notes. Billing sits in the same workflow, with practice management, real-time insurance eligibility, claims management and patient payments. Beyond that, Tebra adds online scheduling, digital intake, HIPAA-compliant two-way messaging and reminders, plus a practice website, listings management and AI replies to reviews, which few EHRs bundle. The weak spot is price transparency: Tebra does not publish plan prices, though its pricing page does explain quote items such as one-time EPCS setup of about $75 per provider and PDMP integration at $500 per facility plus about $50 per user a year. Ask for the full quote, including those items, before you sign.

    Strengths

    • +EHR, billing and patient-facing tools share one platform and login
    • +Marketing and reputation tools are included in the same system
    • +Pricing page explains EPCS and PDMP setup costs that appear on a quote

    Drawbacks

    • −No published plan prices; a quote is required
    • −Marketing tools add scope that a small clinic may not need
    • −EPCS and PDMP setup carry their own one-time and yearly fees

EHR Bundle from $126 per provider per month on an annual contract billed monthly; Full Suite with billing $335 per provider per month

Quoted from the RXNT pricing page on November 6, 2026: $126 / month Annual contract billed monthly, per provider

RXNT is the best value here for practices that want to see prices before a sales call. Its pricing page lists every bundle: the EHR Bundle at $126 per provider a month covers EHR, prescribing, scheduling, the patient portal and Ambient IQ, the PM Bundle at $213 adds medical billing, scheduling and mobile apps, and the Full Suite at $335 includes every RXNT product. The PM Bundle and Full Suite include unlimited claims, which makes billing costs easy to forecast. Ambient IQ, RXNT's ambient listening tool, records visits and drafts the clinical note. All bundles sit on an annual contract billed monthly, EPCS needs $85 a year for ID proofing, and extras such as texting and paper statements are priced separately. The plans are software, so a practice still needs someone in-house or an outside biller to work claims and denials.

    Strengths

    • +Every bundle price is published in US dollars
    • +Full Suite and PM Bundle include unlimited claims
    • +Products can be bought a la carte or as one suite

    Drawbacks

    • −Plans need an annual contract
    • −EPCS ID proofing costs an extra $85 a year
    • −Some add-ons such as texting and statements are billed on top

    Price

Free edition for under 50 encounters a month; encounter-based plan; Provider Plan $200 per provider per month

Quoted from the CharmHealth pricing page on November 6, 2026: The $200 Provider Plan includes an allowance of 1000 SMS/Text Messages per month

CharmHealth is the cheapest way into a full EHR with billing. Practices with fewer than 50 encounters a month can use the free edition and buy only the add-ons they need, and once a practice crosses that threshold Charm moves it to the Encounter Plan, priced by encounters rather than providers. The Provider Plan costs $200 per provider a month and includes 1,000 texts and 500 faxes,. A $350 plan doubles those allowances. Billing covers claims management and eClaims through Optum, and CharmHealth runs weekly live sessions on billing, eClaims and onboarding. The interface is functional rather than polished, the free tier is limited, and per-message overages can add up for busy offices, so estimate your text and fax volume before choosing a plan.

    Strengths

    • +A free edition lets small practices start at no cost
    • +Encounter pricing does not depend on the number of providers
    • +Weekly live training sessions on billing and onboarding

    Drawbacks

    • −The free edition has a limited feature set
    • −Usage overages for texts and faxes are billed per message
    • −Annual discounts require contacting sales

Elation Health is the best fit for independent primary care. Its platform joins the EHR and billing, with real-time eligibility checks, ERA posting, claims processing, and coding and validation help after each visit. Clinical tools are designed around primary care workflows, and Note Assist is one of several AI documentation features that Elation says are now included at no extra charge. Patients use Patient Passport for records and messaging, and scheduling, intake and telehealth sit in the same system. Elation also supports practices paid through insurance, membership fees or a mix, with native membership management for direct primary care and population health tools for value-based contracts. The trade-offs are price and scope: Elation publishes no prices and quotes each practice, and its specialty focus is narrower than athenaOne or Tebra, so surgical or procedure-heavy practices should look elsewhere.

    Strengths

    • +Built specifically for primary care specialties
    • +Supports insurance, hybrid and membership-only payment models
    • +Over 300 integrations listed

    Drawbacks

    • −No published prices; pricing is a personalized quote
    • −Narrower fit for specialties outside primary care

EHR Essential from $229 per provider per month; EHR + PM Essential $349 per provider per month

Quoted from the Practice EHR pricing page on November 6, 2026: $229 /mo per provider Start Essential

Practice EHR publishes clear tiers, which makes it easy to cost out. The Essential EHR plan at $229 per provider a month includes progress notes, a mobile app, patient portal, scheduling, e-prescribing, telehealth, a kiosk, API integration and patient self-scheduling. For billing, the EHR + PM Essential plan at $349 adds claim submission and denial management, and the Pro tiers at $379 and $499 add an AI scribe, AI scanner, AI coding and AI fax, plus an AI claim editor on the PM side. Practices that want to hand off billing can quote an RCM plan that adds payment posting, statements, prior authorization and credentialing. Every plan includes free onboarding and migration help and unlimited patients. Compared with RXNT, the EHR plus billing entry price is close, but RXNT's suite includes ambient documentation at its lower price, while Practice EHR keeps AI features on Pro.

    Strengths

    • +Prices for EHR and EHR + PM tiers are published
    • +Free onboarding and migration assistance on all plans
    • +Unlimited patients on every plan

    Drawbacks

    • −AI tools start on the Pro tier at $379 or $499

eClinicalWorks has one of the deepest feature sets on this list and a very large installed base: the company reports more than 180,000 physicians and nurse practitioners using its EHR across 110,000 facilities. The V12 EHR includes AI features, Sunoh.ai provides an ambient AI scribe, and the healow platform adds patient self-scheduling, check-in, telehealth and secure messaging. healow Genie, an AI contact center, answers patient calls around the clock. For billing, eClinicalWorks sells practice management and a revenue cycle management service, and it cites a first-pass acceptance rate of 98 percent or higher. The trade-off is complexity: many of these capabilities are separate products, so a small practice has more to evaluate, configure and train on. We could not load USD pricing for eClinicalWorks from our location, so request a written quote that lists every add-on.

    Strengths

    • +Very large user base and broad specialty coverage
    • +AI scribe and AI contact center from the same vendor
    • +RCM service available for practices without billing staff

    Drawbacks

    • −Pricing could not be confirmed in USD from this location
    • −Patient engagement, AI scribe and RCM are separate products to evaluate
    • −Fewer self-serve plan details than vendors with published tiers

DrChrono packages its EHR, patient engagement, practice management and billing tools into three plans. Even Foundation includes custom billing codes, an integrated claim management tool, ERA auto-posting, autogenerated superbills and CMS 1500 forms, plus a check-in kiosk, patient portal and app, reminders and unlimited staff accounts. Growth adds revenue cycle software and automated workflows, and Premium adds advanced AI and premium add-ons. The clinical side covers specialty templates, text-expanding macros, integrated photo and drawing tools and mobile EHR apps, with the EverHealth Scribe and medical speech-to-text sold as add-ons. Each new customer gets a dedicated implementation specialist and training. Costs are less clear than they look: plan prices need a quote, eligibility checks and claims are charged per use, and the pricing page lists overage fees for texts, faxes, calls, paper claims and statements.

    Strengths

    • +Billing tools are included from the Foundation plan
    • +Unlimited staff accounts
    • +Dedicated implementation specialist and training team

    Price

From $199 per provider per month with a required annual commitment, billed monthly; 3 signing staff licenses included

Quoted from the Practice Fusion pricing page on November 6, 2026: starting at just $199/month per provider with a required annual commitment

Practice Fusion is the easiest EHR here to price: $199 per provider a month with a required annual commitment, billed monthly, including three signing staff licenses and unlimited non-signing staff. A two-week free trial lets a practice test charting and e-prescribing first. The EHR is straightforward and suits independent practices that mostly need clinical documentation. Billing works differently from the suites above. Instead of practice management software built into the EHR, Practice Fusion offers billing services run by a dedicated team, covering payer enrollment, payer compliance, claims scrubbing, same-day submission, credentialing and financial reporting. That helps a practice without a biller, but practices that want to work their own claims in-house will be better served by RXNT, CharmHealth or DrChrono. Note that taxes are not included in the published price.

    Strengths

    • +Published price that includes staff licenses
    • +Free 2-week trial
    • +Short onboarding with a dedicated team

    Drawbacks

    • −Billing is a service rather than built-in practice management software
    • −Requires an annual commitment

    Price

Hosted EHR from $229 per month; locally installed EHR $249 per month; EHR + Practice Management bundle $499 per month

Quoted from the Amazing Charts pricing page on November 6, 2026: Electronic Health Records (EHR) - Hosted $229/month

Amazing Charts is an EHR for independent practices with plain published pricing. The hosted EHR costs $229 a month with automatic updates, e-prescribing, templates and US-based support, and a locally installed version costs $249 a month for practices that want the software on their own hardware. For billing, the bundled EHR and practice management plan costs $499 a month and adds an integrated clearinghouse, payment posting, a claims manager and reporting. Revenue cycle management, patient engagement, population health and lab and radiology interfaces are sold as additional services. The feature set is narrower than the cloud suites above, with less built-in AI and patient self-service, and the billing bundle is priced higher than RXNT's Full Suite. It remains a reasonable choice for small practices that value a local install and simple pricing.

    Strengths

    • +Published prices for hosted, installed and bundled plans
    • +A locally installed option for practices that want on-premises data
    • +US-based support

    Drawbacks

    • −The bundle with billing costs $499 a month

What to check before you buy

Decide who will work the claims

Some vendors sell software and leave billing to your staff, while others sell billing services as well. athenaOne includes billing experts, coders and an authorization team, and Practice Fusion offers billing services instead of built-in practice management. RXNT, CharmHealth and DrChrono give you the tools to bill in-house.

Compare pricing models, not only headline prices

Flat per-provider fees such as RXNT's $335 Full Suite or Practice EHR's $349 EHR + PM plan are easy to forecast. Percentage of collections, as with athenaOne, rises with revenue. CharmHealth prices by encounters or providers, and DrChrono charges per claim and per eligibility check on top of its plans.

Check that billing is in the plan you are quoting

Entry plans often cover only the EHR. RXNT's EHR Bundle at $126 and Practice EHR's Essential EHR at $229 do not include claim submission, and Amazing Charts adds billing only in its $499 bundle. Ask for a quote that names the clearinghouse, ERA posting and statements.

Budget for e-prescribing of controlled substances

EPCS needs identity proofing and often a state PDMP connection. RXNT charges $85 a year for EPCS ID proofing, and Tebra lists about $75 per provider for EPCS setup and $500 per facility for PDMP integration. Ask every vendor for these line items.

Weigh AI documentation tools

Ambient scribes now come from most vendors: RXNT's Ambient IQ, eClinicalWorks' Sunoh.ai, Tebra's AI Note Assist, Elation's Note Assist and Practice EHR's AI scribe on Pro. Check whether the scribe is included or an add-on, and test it on your own visit types during a trial or demo.

Common questions

What is the best EHR for a small independent practice?
athenaOne ranks first for small practices that want billing support from the vendor, priced as a share of collections. Tebra is the best all-in-one platform for EHR, billing and patient marketing, and RXNT is the best value with a published $335 per provider Full Suite that includes billing.
How much does EHR software cost for a small practice?
Published prices on this list run from a free CharmHealth edition for under 50 encounters a month to $499 a month for Amazing Charts' EHR and practice management bundle. Practice Fusion costs $199 per provider a month, RXNT's EHR Bundle $126 and its Full Suite with billing $335. athenaOne charges a percentage of collections.
Is there a free EHR with billing?
CharmHealth offers a free edition with a limited feature set for practices under 50 encounters a month, and practices can add paid add-ons. Above that volume Charm moves the practice to its encounter-based plan.
Should a small practice choose percentage-of-collections or flat-fee EHR pricing?
A percentage model, such as athenaOne's, usually includes billing staff support and ties the vendor's pay to collections, which helps practices without a biller. A flat per-provider fee, such as RXNT or Practice EHR, is easier to forecast and often cheaper once collections grow, but your own staff work the claims.
Which EHR is best for direct primary care?
Elation Health is built for primary care and includes native membership management for direct primary care, and it supports insurance, hybrid and membership-only payment models on one platform.
